A. All application should be submitted to the Chapter President for their perusal. The chapter President will then make the recommendation to the Committee on Membership Credentials, Accreditation & Re- Classification (CMCAR) thru the National Secretariat. B. Submitted applications will be processed within 15 days upon receipt by the National Secretariat. C. Approved applications will be mailed, faxed or emailed to the Chapter president. D. Induction will be conducted in the Local Chapter in the presence of the National President and/or member of Executive Officer and chairman and/or vice chairman of the CMCAR.

Documentary Requirements for PAMS’ Membership: [a] Fully-accomplished Application Form [b] Recent photos: ID pictures (1 copy each) 2” x 2” and 1” x 1” (either colored or black & white) [c] Membership Fee of Three Thousand pesos (PhP 3,000.00) Three (3) sets of photocopies (with the original or certified true copies for presentation purposes) of the following for submission to PAMS Committee on Membership Credentials & Accreditation: [d] Diploma from Medical School [e] Certified True Copy of Medical Specialist Diploma and/or Certificate of Completion of Residency Training [f] Certificate of Medical Specialist’s Qualifying Examination (DOH) or Fellow/Diplomate Certificate from other specialty society or medical organization [g] Photocopy of Updated PRC (Professional Regulation Commission) Card [h] Photocopy of Updated PMA (Philippine Medical Association) Card
